Cancelations, deposits & rescheduling

*I understand that all appointments require a 50% deposit that is non-refundable.

As per the cancellation and rescheduling policies, I agree to provide the required corresponding notice or my deposit will be forfeited and applied toward the cancellation fee.

If I have provided sufficient notice (more than 48 hours), my deposit can be transferred to a new appointment date and time, appointment type, or can be used toward the purchase of retail products.

*I agree to the cancel and reschedule policy. 

I understand that when I book with Cocoa Lash, Brow & Makeup Artistry that the time is especially reserved for me, hence if I do not provide the minimum notice as per below, I will be liable for the corresponding fees. Any deposit paid will be applied toward the following fees and any remaining balance will be billed to me via email and payable immediately;

4-48 hours notice = 50% of the appointment fee applies

0-4 hours notice = 100% of the appointment fee applies 

No-shows = 100% of the appointment fee applies and future appointments may be refused.

Lashes

*I consent to the procedure of having synthetic eyelash extensions attached by cyanoacrylate adhesive to my natural eyelashes. 

*I understand that there are risks associated with having eyelash extensions attached. I understand that the adhesives and adhesive removers may cause, but not be limited to, watering of my eye/s, redness, itching, and/or irritation.

*I agree to arrive to each appointment with clean, make-up free eyelashes and eye area. Failure to do so will result in necessary cleaning by my eyelash technician, hence less extensions applied. Retention is not guaranteed if I arrive with eye make-up. 

*I agree to follow the provided aftercare instructions and understand that if i do not, it will compromise the health and longevity of my eyelash extensions and natural lashes.

Brows

*LAMINATION  -  I consent to the procedure of having my brows reshaped by process of lamination. That is, chemical solutions are applied to both my eyebrow hairs and skin in order to achieve a new eyebrow shape, as discussed with my Brow Artist.

*TINT  -  I  consent to the procedure of having my brows tinted by process of  dye. That is, a chemical solution is applied to both my eyebrow hairs and skin in order to achieve a new eyebrow colour, as discussed with my Brow Artist.

*WAXING -  I  consent  to  the  procedure  if  having   my  brow  hairs  removed.  That  is,  hot  wax  is  applied  to  both  my  eyebrow  hairs  and  skin.

*I understand that there are risks associated with eyebrow  services  such  as  redness,  irritation,  superficial  burn  to  the  skin  and  reduction  in  future  hair  growth.  

*I agree to arrive to each appointment with clean, oil-free, make-up free, self-tan-free brows and eye area. Failure to do so will result in the necessary thorough cleaning by my Brow Artist, hence reducing solution timing, effectiveness & longevity of the lamination and/or tinting results.

*I agree to follow the provided aftercare instructions and understand that if I do not, it will compromise the shape & longevity of my eyebrows.

All Services

*If at any time during the service I experience discomfort I will inform my technician immediately so as he/she can attempt to rectify the discomfort. 

*I understand that it is my responsibility to remain still and keep my eyes closed during the entire procedure until otherwise advised by my  technician.

*I have disclosed any and all health conditions that may affect my eyes, skin and hair, as well as any current medication I am taking. I agree to disclose any newly diagnosed health conditions and medication being taken whilst i remain a client 

*I consent to photographs of my eye area and/or face to be taken during my appointment for the purposes of internal use (consultation, evaluation and before/after records) by Cocoa Lash & Brow Artistry. 

**Optional. I consent to photographs of my eye area and/or face to be taken during my appointment for public marketing use on Cocoa Lash & Brow Artistry's website and social media platforms.

*I  agree  to  notify  Cocoa  Lash,  Brow  &  Makeup  Artistry  within  24  hours  if  i  am  not  happy  with  my  treatment.  Every  attempt  will  be  made  to  come  to  a  suitable  solution  however  refunds  are  not  provided  for  services  already  performed.

*I agree that I have read and fully understand this entire consent form. I am of sound mind and fully capable of executing this waiver for myself 

*I agree that by ticking and therefore agreeing to each term listed on this form, that I release Cocoa Lash & Brow Artistry from any claims or damages of any nature and that this agreement will remain in effect for initial and future services performed.
